The chest contains Limited Edition content - you either have to have purchased a copy of the LE game or been given a code. If Lionhead are still handing them out on the forums then you can try registering there and finding out how to get one.
Once you've entered the code you should be able to open the chest. Just make sure you follow the instructions given with the code, if any.

You get one doll per world, as it were, so if you get second prize at the Westcliff Shooting Range, you get the doll. Then you have to have the other dolls through trading via Xbox Live - the Doll Exchange thread in the Trading forum on here has plenty of people willing to share/trade dolls.
Either that or see someone else get the achievement if you want to simply unlock it without keeping any dolls.

If you watch a story on the website, items will be placed in the chest depending on which ending you choose for the story.
If you do go to the website, I suggest you watch the story 3 times and choose all 3 endings, then go into your game and open the chest.

See now I'm confused. People say that's where you get Hal's armor and everything and others say you have to go to the lionhead website and get the stuff from there.
Is it both ? Or one or the other?

It only worked in IE for me. If you havn't already try it in that.

Aw! try signing into your gamertag before clicking "play" I don't know if that will help at all but its what I did.
Just to clear it up for you. The Hal's armour is picked up from that chest after entering the LE code. The puppet game prizes (from fable2.com) also appear in the chest. If you got both the LE and fable2.com stuff at the same time all the stuff will be in there...well apart from sometimes the fable2.com stuff doesn't actually appear in the chest but in the game world...but thats another story
