Sanctuary glitches

I was having trouble with save files, some items disappearing, my hero sliding around instead of running and then not working at all, stuck in some endless fall animation.

So I started a new hero and halfway through the game, I went into my sanctuary and saw this "hole" into the rooms behind the doorways through the Road to Rule wall.

Then I was joined by a friend over xbox live and when we went to his sanctuary, all his items were held by statues with the outfit sets on. I apologize for the shots, but I had to try to capture this with my iphone and old tv.

Has anyone else seen this? My friend didn't see it, just me on my side....

It's the connection problem with either you or your friend, if both player have good connection, then the game is fun to play co-op online, but if the connection got drop on either end, then you will see some weird stuff going on.
