You receive the hat during the Rescuing Charlie quest

HOW TO GET CHARLIES HAT (SPOILER):

Spoiler

In order to obtain this hat, you must let Charlie die after opening the coffen inside when the enemies attacks him. You then can loot his body and obtain his hat. Wear this hat before you leave the cave to impersonate Charlie. Once you leave the cave you will come across his grandmother waiting outside the cave and think you are Charlie and give you his birthday gift as well as allow you to keep his hat.
