Unresponsive World

My brother went to Bowerstone Market. He started killing people, and the guards don't even noticed. The people don't get scared or anything. Then they started teleporting everywhere. Then he shot a door and it just stayed normal. Then when he stepped in the door, it suddenly broke into pieces. His dog also just sits. He went to another region, and enemies don't do anything.

Is this in Co-Op or Single Player? If Co-Op, online or localized (same XBox)?
