My nuetral character Arrowdodger has a big (but not unnaturaly so) scar just over his right eye... and by big, I mean long! Wicked awesome... it's almost a perfect line from his brow half way (or more? I can't remember precisely...) down his cheek. Ohhh! And my good character, Atreyu Spect, has a huge scar from his left shoulder to his right hip!
Can't you avoid scars by wearing armor ?

Nope! That's why they're called scars, not cuts or boo-boos :3

Probably lack of Armor, toughness + balverines.

but if you are using physical shield, theres no scar or a single cut

You can remove scars with modding I thought. I haven't tried it yet though
